Self certification for the first seven days, only then will you be required to obtain a sick line. Please contact the surgery to order a renewal sickline or to make an appointment to discuss the reason for you being off work with the Dr.
Please note that all sicklines are to be collected, previously we had offered emailing as part of covid precautions., the email service has now been withdrawn. If you are very ill and can not come and collect the slickline, we will in certain circumstances allow the emailing of them.
